Membership dues for 2023 are $45. This form must be completed in its entirety. All information will be used in our annual membership directory, unless you request otherwise. To request that any information be omitted from the directory, type OMIT after your information for each line you’d like omitted. While all Cedar Creek residents are included in the directory, only paid members will have access to it. If you choose not to join CCCA, please complete and submit this form anyway, to ensure accuracy. Please note that we never share or sell your information with anyone.

Checks made out to CCCA and mailed or delivered to the Treasurer, Ken Portier, at 160 Spruce Valley Road.
